On-the-job Injuries & Illnesses - IUN campus

Employees at IUN must go (if not being transported by ambulance) to one of two Methodist Hospital clinic locations:

On-the-job Injuries & Illnesses - IUPUI campus

IUPUI staff must go to Occupational Health Services, Coleman Hall, for treatment of on-the-job injuries and illnesses. If the health center is closed or if the injury is an emergency staff should go to the University Hospital Emergency Room.

At OHS you'll also be given a slip of paper that explains some of the obligations you have if you've been placed off work or on restricted duty. You can see that form by looking here. Please contact Angie Stierwalt or Hans Van Leeuwen if you have any questions.

When there is an injury/illness your supervisor must be notified and an authorization-to-treat form completed. The required form can be found at https://rmweb.indiana.edu/ORM/secure/WCAuthEntryIUPUI.cfm.

The Injury/Illness Form, https://rmweb.indiana.edu/orm/secure/ads/Workcomp/Injuryreport/InjuryReport.cfm should be filled out by a supervisor (or responsible office) and submitted. A printed copy must be signed by the employee and supervisor and mailed to Risk Management, Poplars 705. Delegation is required through the office of Risk Management. To obtain delegation, please refer to Important Notes - Worker's Compensation below.

On-the-job Injuries & Illnesses - IUB campus

Employees (for those injuries and illnesses covered by workers compensation) shall go to the Promptcare/Bloomington Hospital Center for Occupational Health (BHCOH) located at 326 Woodcrest Drive (the corner of East 3rd Street and Woodcrest Drive). This is east of High Street. Look for the IU/Rebound entrance.


Effective January 12, 2003 these procedures also include non-employee student volunteers in laboratory settings for at least the initial visit after an injury in a laboratory setting. I.e., if both employees and student volunteers are injured in an incident in a laboratory setting it is no longer necessary to send some people to BHCOH-Promptcare and some people to the Health Center. All should go to Promptcare and the webform linked below should be completed for each person.

ORM claims personnel will handle the non-employee cases and will contact those injured about continued treatment.


The clinic's hours are 8:00 AM - 8:00 PM seven days a week.

When there is an injury/illness your supervisor must be notified and an authorization-to-treat form completed. This form can be completed on the web or can be downloaded and completed as a paper form. The supervisor (or responsible office) should send a copy of this form with the worker (for the initial visit) or complete the web form while the worker is being transported to the clinic.

If the web form is used Risk Management and the occupational health unit will receive copies. If the paper form is used a copy should be sent with the employee to the clinic and a copy immediately faxed to Risk Management.

There is no change in after hours procedures (now 8:00 pm - 8:00 am) or critical cases (where an ambulance is called). Go to the Bloomington Hospital Emergency Room.

Important Notes - Workers Compensation

Supervisors

Two required pieces of information when you submit the authorization form are your telephone number and your fax number. In some case one or both may actually be a departmental number rather than a direct line to you. The telephone number is required so the occupational health physician can contact you, if necessary, to discuss the employee's duties so they can better determine what work restrictions should be in place. The fax number is needed so they can immediately (as the employee finishes their visit) fax to you their job restrictions (if any). If a job description is not on file with BHCOH then a copy of the employee's job description should immediately be faxed to BHCOH.

Delegation

Delegation is required to access the Occupational Injury/Illness form. Departmental management will delegate supervisory level and above to have access to the Occupational Injury/Illness form. Delegates are required to file an Occupational Injury/Illness form for all reported on the job injuries regardless whether professional medical attention or lost time is indicated. To obtain delegation, please call 812-855-4847.

Employees

We receive many telephone calls from employees who are not clear about what they need to do after an injury.

First, of course, is immediately report the injury to your supervisor. The supervisor will then give you instructions about seeking treatment.

When you have finished your visit at BHCOH you will be given (among other things) a slip of paper with your work restrictions. A copy of that slip is also faxed to your supervisor and a copy to us in Risk Management. Unless the physician has put you off work you immediately return to your department and report to your supervisor. If you have been given work restrictions your supervisor will begin the process of finding you restricted duty.

Return To Work

  1. Following receipt of medical treatment the employee will be given a Worker’s Compensation visit form that may list work restrictions that the employee will need to follow. Upon leaving the medical facility, the employee must take the form back to their supervisor for evaluation of transitional work that may be available. If transitional work is not available through your home department, your departmental coordinator is required to call the Worker’s Compensation Unit at 812-855-4847 before the employee leaves campus.


  2. The following are three possible outcomes for an injured employee following medical Treatment.
    1. The employee is released to return to work.

    2. The employee is released to work with temporary restrictions, sometimes referred to as “light duty” or “transitional duty”. If the university can provide work for the employee, the employee is brought back to work with restrictions: if the university cannot provide work, the employee remains off from work

    3. The employee is not released to return to work and remains off work.

  3. If the attending physician gives the employee work restrictions and the university can provide work for the employee within these restrictions, the employee is not entitled to receive temporary total disability benefits. If the employee declines to participate in the return to work program, the employee will be placed on an FMLA leave, if eligible.

    1. The job pool allows employees who have been released to work with temporary restrictions and whose departments cannot provide them with transitional work an opportunity to continue to work.

    2. Employees who choose to participate in the return to work program will be given a temporary job on campus outside of their home department

Long Term Monitoring

A number of employees are on long term monitoring for health related reasons. If you are one of these employees:

You will receive a letter from Risk Management sometime in the next few weeks. This letter will have some information about the change in occupational health providers and a medical records release form. Within two to three weeks after you return the records release form to us you will receive a letter from the Bloomington Hospital Center for Occupational Health (BHCOH) scheduling your next monitoring visit. If you have a scheduled visit before you receive that letter, go to the Health Center as you have done in the past.

Other occupational health services

Other related occupational health services will also be provided at BCHOH. Examples include services required for respirator fittings and drug screenings. The offices responsible for arranging those services will notify the employees where to go.
